Quantitative vs Qualitative Tests: Why the Difference Matters

A qualitative test answers "is it there?" (positive/negative). A quantitative test answers "how much?" (a concentration). In veterinary diagnostics, that difference changes clinical decisions.

Three Clinical Scenarios Where Numbers Matter

1. Staging kidney disease

SDMA is a number (µg/dL). IRIS CKD staging maps SDMA values to stages 1–4. A qualitative "kidney problem: yes" cannot stage the disease — a number can.

2. Monitoring treatment response

A dog with pancreatitis has a CRP of 85 mg/L. After 3 days of treatment, CRP drops to 20 mg/L. A qualitative test would still read "positive" — only a quantitative test shows the patient is improving.

3. Making threshold decisions

NT-proBNP > 900 pmol/L in a dyspneic dog shifts the differential toward cardiac disease. This is a threshold decision — impossible without a number.

When Qualitative Is Enough

Qualitative strips remain appropriate for pure screening where yes/no suffices and cost dominates — for example, a first-line CPV antigen screen.

FAQ

Can a quantitative test be used for screening? Yes — a quantitative result always contains the qualitative answer (above/below cutoff) plus the concentration.

Why not always use quantitative? Cost and complexity. Quantitative requires an instrument. Where a number doesn't change management, qualitative is sufficient.
